Anxiety

Anxiety Disorders

Generalised anxiety • Panic • Social anxiety • Phobias

Anxiety is the most common mental-health concern we treat at Realize, and one of the most rewarding to help with, because relief usually comes faster than patients expect. Everyday worry becomes an anxiety disorder when it is persistent, disproportionate, and starts to interfere with sleep, work, relationships or health. Patients describe a mind that will not stop racing, physical symptoms like a pounding heart, chest tightness, breathlessness, tremors and stomach upset, and a growing tendency to avoid situations that trigger discomfort. Panic disorder brings sudden waves of intense fear that feel like a heart attack; social anxiety turns ordinary interactions into ordeals; generalised anxiety keeps the mind on constant alert. Our care combines a full psychiatric assessment to rule out thyroid, cardiac or hormonal causes, evidence-based Cognitive Behavioural Therapy, exposure work for phobias and panic, mindfulness training, and, when needed, modern medications such as SSRIs or short-term supportive treatment. Most patients see meaningful change within 4–8 weeks and lasting remission with structured follow-up.

Depression

Depression

Persistent low mood • Loss of interest • Fatigue

Depression is far more than sadness, it is a medical illness that changes how the brain processes reward, motivation, sleep and appetite. Patients describe a persistent low mood, loss of interest in things they once enjoyed, low energy, disturbed sleep, changes in appetite, poor concentration, feelings of worthlessness or guilt, and in severe cases, thoughts of self-harm or suicide. Left untreated, depression damages relationships, careers and physical health, and dramatically increases the risk of substance misuse. The good news is that depression is one of the most treatable psychiatric conditions when addressed early. At Realize, we begin with a full assessment to rule out medical causes (thyroid, anaemia, vitamin deficiency), identify the subtype (major depression, persistent depression, postpartum, seasonal, bipolar depression), and screen for co-existing anxiety, trauma or substance use. Treatment combines evidence-based psychotherapy (CBT, IPT, behavioural activation), modern antidepressants where indicated, family support, and lifestyle interventions covering sleep, exercise, nutrition and sunlight. For severe or treatment-resistant depression, advanced options such as combination therapy or referral for rTMS are discussed. Most patients experience meaningful improvement within 4–6 weeks.

Bipolar

Bipolar Disorder

Mood stabilisation • Long-term wellness • Family support

Bipolar disorder is a chronic mood condition marked by episodes of depression alternating with episodes of mania or hypomania, periods of unusually elevated mood, high energy, reduced sleep, rapid speech, impulsive decisions and sometimes irritability, grandiosity or reckless behaviour. Between episodes, most patients function well, but untreated bipolar disorder can devastate relationships, careers and finances, and carries a serious risk of substance misuse and suicide. The good news is that with expert psychiatric care, most patients achieve long stretches of stability and full, meaningful lives. At Realize, our approach begins with careful diagnosis, bipolar disorder is often mistaken for depression, ADHD or personality disorder for years before it is correctly identified. Once diagnosed, treatment combines mood-stabilising medications (lithium, valproate, lamotrigine or modern atypical antipsychotics), psychoeducation for the patient and family, structured therapy (CBT, IPSRT) focused on sleep-wake regularity and early-warning recognition, and long-term follow-up. Family involvement is central because episodes are often first detected by loved ones. With sustained treatment, relapse rates drop dramatically and patients regain confidence in themselves and their future.

OCD

Obsessive Compulsive Disorder (OCD)

Intrusive thoughts • Compulsions • Evidence-based ERP therapy

OCD is a distressing but highly treatable condition in which the mind is hijacked by unwanted intrusive thoughts (obsessions) and repetitive behaviours or mental rituals (compulsions) performed to reduce the anxiety these thoughts cause. Common themes include contamination fears with excessive washing, doubt with repeated checking, symmetry and ordering, unwanted taboo thoughts, and religious or moral scrupulosity. Patients often feel deeply ashamed of their thoughts, hide symptoms for years, and try to reason their way out, which unfortunately makes OCD stronger. At Realize, we treat OCD as a medical, brain-based condition rather than a character flaw. Assessment identifies the specific OCD subtype and screens for co-existing depression, anxiety or tic disorders. The core treatment is Exposure and Response Prevention (ERP), a specialised form of CBT with the strongest evidence base for OCD, delivered by therapists trained in the technique. SSRIs at OCD-specific higher doses are used alongside therapy for moderate to severe cases. With committed treatment, most patients experience 60–80% symptom reduction and reclaim their time, freedom and peace of mind.

PTSD

Post-Traumatic Stress Disorder (PTSD)

Trauma-informed care • Safety • Recovery

PTSD develops after exposure to a life-threatening or deeply distressing event, an accident, assault, medical emergency, natural disaster, combat, childhood abuse or the sudden loss of a loved one. The mind and body remain stuck in a state of alarm long after the danger has passed, with intrusive memories or flashbacks, nightmares, hypervigilance, avoidance of reminders, emotional numbing, sleep disturbance and mood changes. Untreated PTSD often leads to depression, anxiety, substance misuse and relationship breakdown. At Realize, we provide trauma-informed care that begins with safety, stabilisation and psychoeducation, followed by evidence-based trauma-focused therapies such as Trauma-Focused CBT, EMDR-informed techniques and prolonged exposure where appropriate. Medications, particularly SSRIs and select adjuncts, help reduce arousal, improve sleep and support therapy. Care is delivered at your pace, in a safe environment, with absolute confidentiality and, for those who wish it, involvement of a trusted family member. Recovery is very much possible; most patients regain a sense of safety, meaning and hope.

Schizophrenia

Schizophrenia and Psychosis

Modern antipsychotics • Rehabilitation • Family support

Schizophrenia and related psychotic disorders are serious but treatable brain conditions in which patients experience altered perceptions (hallucinations), fixed false beliefs (delusions), disorganised thinking, reduced motivation and social withdrawal. Onset is typically in late adolescence or early adulthood, and early intervention makes an enormous difference to long-term outcome. Modern psychiatric care has transformed the prognosis of schizophrenia, today most patients live meaningful lives, complete education, work, marry and raise families with the right treatment. At Realize, our team includes psychiatrists with specific experience managing severe mental illness, including at the Institute of Mental Health, Erragadda. We provide careful diagnostic assessment, modern atypical antipsychotic medications with lower side-effect profiles, long-acting injectables where adherence is a challenge, structured psychosocial rehabilitation, family psychoeducation, and long-term follow-up. Care can be inpatient during acute episodes and outpatient for maintenance. Confidentiality, dignity and respect are central to everything we do.

ADHD

Attention Deficit Hyperactivity Disorder (ADHD)

Children • Adolescents • Adults

ADHD is one of the most common and most under-recognised neurodevelopmental conditions, affecting children, adolescents and adults. Core features are difficulty sustaining attention, distractibility, forgetfulness, disorganisation, impulsivity and (in some) hyperactivity. In children it shows up as poor school performance, restlessness and difficulty following instructions; in teens as academic decline, risk-taking and mood struggles; in adults as chronic underperformance, procrastination, time-management failures, difficulty completing tasks and strained relationships. Untreated ADHD is a major driver of anxiety, depression, substance misuse, gaming and internet overuse, and career or educational underachievement. At Realize, we provide comprehensive ADHD assessment, clinical interview, standardised rating scales, screening for co-existing conditions and, where indicated, psychological testing. Treatment is highly individualised and combines psychoeducation, structured behavioural strategies, parent training or coaching for adults, school liaison for children, CBT for adult ADHD, and, where clinically appropriate, modern medications (stimulant and non-stimulant options) which are safe, effective and reversible. Most patients see life-changing improvement within weeks of appropriate treatment.

Autism Spectrum Disorder is a neurodevelopmental condition that affects how a person communicates, relates to others and experiences the world. Children with autism may show delayed speech, limited eye contact, difficulty with social reciprocity, repetitive behaviours, restricted interests, sensory sensitivities and preference for routine. Presentation varies widely, from children who need substantial support to highly capable individuals with unique strengths. Early identification and intervention transform outcomes. At Realize, we provide structured developmental and psychiatric assessment, guidance on evidence-based interventions (Applied Behaviour Analysis, speech and language therapy, occupational therapy, structured teaching), parent coaching, school liaison, and management of co-existing conditions such as ADHD, anxiety, sleep problems, seizures or aggression. Care is respectful, family-centred and paced to your child's needs, with the goal of building communication, independence and quality of life.
